New Patient Registration
To register with our practice please complete and sign a registration form, issued at the surgery, or download one from the “Registering Online” page. You will need to bring two forms of ID, at least one with a photo, to the surgery. You will not be registered until you have done this and signed the registration form.
If your application is accepted the West Norfolk CQC Trust will notify you in writing of your acceptance.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
